We understand the critical need for stable housing and supportive services for individuals experiencing homelessness or housing instability. That's why we offer comprehensive Housing Transition Navigation services tailored to meet the unique needs of each individual or family.
Our Housing Navigators work closely with members to develop personalized housing plans, taking into account factors such as housing, food, healthcare, and safety.
We prioritize individuals who are at-risk of homelessness due to major barriers to housing stability, including those with serious chronic conditions, serious mental illness, or substance use disorders.
Through our program, eligible members can receive ongoing support for as long as needed, with an initial authorization for 12 months and extensions available based on medical necessity. Our goal is to empower individuals to secure permanent housing and access the supportive services they need to thrive in their communities.
